29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: (a) This violation occurred on or about November 26, 2019 when an employee working on top of a mold and repairing a broken water cooling fitting was exposed to a fall hazard of approximately seven feet to the concrete floor below.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) This violation occurred on or about November 26, 2019 when the employer did not develop energy control procedures for the control of pressurized nitrogen gas on the new style mold. Employees working and repairing a water leak under a cover plate attached to the backing plate of the mold were exposed to a struck-by hazard from four cylinders filled with nitrogen gas and exerting a force of approximately 1,000 kilograms.
