29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    a.  On or about November 26, 2019, at the above addressed jobsite, an employee wore loose fitting gloves (Boss - Munk), while operating the Skilsaw Worm Drive Circular Saw Model # SPT77W Serial #612003985. A glove came in contact and was pulled into the rotating saw blade, resulting in a partial finger amputation. Employees were not trained in the proper use of circular saws and proper glove selection when using such saws.    In accordance with 29 CFR 1913.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abetment is complete must be included with your certification.
