29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:  On or about December 4, 2019, employees performing sewer and water installation at a residential property in a 5.7 foot deep trench were exposed to cave-in hazards when the employer failed to ensure the employees were protected by an adequate cave-in protection system.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
