29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): a) On or about December 5, 2019, and at times prior, the employer did not certify that the fork lift operator had been trained. Certification must include the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation, and the certification shall be updated at least once every three years. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
