29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function.    a.  On or about December 4, 2019, at the above addressed jobsite, employees conducting roofing operations were exposed to fall hazards of thirty (30) feet eight (8) inches, due to the safety monitor having other responsibilities, such as but not limited to, roofing operations.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval:    a.  On or about December 4, 2019, at the above addressed jobsite, an employee was operating a Caterpillar GP-15 LP Forklift with a LP-gas tank that was not in accordance with the manufacturer's specifications. This practice could affect the safe operation of the equipment and the manufacturers written approval was not obtained. The employees were thereby exposed to hazards associated with fire, explosion, and burns.

On or about December 4, 2019, at the above address jobsite, the employer did not ensure the employee operating the GMC C7500 Series 500C Boom Truck Crane was trained, certified/licensed, and evaluated in accordance with the requirements of 29 CFR 1926.1427, thereby exposing employees to struck by hazards.
