The hospitalization occurred on or about December 7, 2019 at approximately 4:00 PM. The hospitalization was not reported to OSHA. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

Kertra Limited DBA Maxi's Grille at the Bel-Aire - On or about December 10, 2019, and times prior thereto: Employees worked with or around hazardous substances including, but not limited to, carbon monoxide, and various cleaners and degreasers during routine cooking and cleaning operations. Employees were not provided effective information and training about hazardous chemicals in the workplace. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
