29 CFR 1910.141(a)(5): A continuing and effective extermination program was not instituted where rodents, insects or other vermin were detected: a.) Kertra Limited DBA Maxi's Grille at the Bel-Aire -On or about January 7, 2020, mouse droppings were observed throughout the kitchen and storage areas. Company did not maintain an ongoing rodent prevention program to prevent the entrance or harborage of mice. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

Kertra Limited DBA Maxi's Grille at the Bel-Aire - On or about January 7, 2020, and times prior thereto : Employer did not ensure hot and cold, or tepid water was provided in each employee restroom. Only cold water was available in the employee restroom, near the kitchen, for hand washing. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
