29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  On or about 12/10/2019, and at times prior, at 16660 Sunset Way, in Botaniko, Weston, Florida, the employer did not develop and implement an accident prevention program that included the following elements: management commitment and employee involvement; worksite analysis; hazard prevention and controls; and safety and health training, to ensure employees of subcontractors and employees of the company were protected from fall hazards when engaged in construction activities.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  On or about 12/10/2019 and at times prior, at 16660 Sunset Way, Lot 64B, at Botaniko, Weston, Florida, employees of contractors were exposed to a fall hazard of 12-feet to ground level when working from scaffolds that were not fully planked.  Gestido Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1247495, citation number 01, item number 01 and was affirmed as a final order on 01/04/2018, with respect to a workplace located at 16479 Botaniko North, Weston, Florida 33326.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about 12/10/2019 and at times prior, at Lot 48B, in Botaniko, Weston, Florida, employees of contractors were exposed to a fall hazard of 17-feet to ground level when working on the edge of a roof, in that; no means of fall protection was being used.    Gestido Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1252956, citation number 01, item number 02 and was affirmed as a final order on 03/08/2018, with respect to a workplace located at 16590 Sunset Way Botaniko South, Weston, Florida.
