29 CFR 1926.304(i)(1):   Each circular had-fed ripsaw was not guarded by a hood whcih completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut.   a)  1706 Paterson Plank Road, Second Floor, North Side,  North Bergen, NJ.  Employees on this multiemployer site were exposed to lacerations or amputations when using the table saw as the blade guard assembly was missing from the 10" Rigid Portable Table Saw with mobile stand.                                                  Violation observed on or about 12/10/2019.

29 CFR 1926.404(b)(1)(i):   Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  a)  1706 Paterson Plank Road,  North Bergen, NJ.  Employees of the general contractor and subcontractors on this multiemployer site were exposed to electric shock as the branch circuits at the site were no longer covered by GFCI protection as the electrician temporarily hooked up permanent power thereby removing the GFCI protection provided by the temporary power system used during construction.                                                  Violation observed on or about 12/10/2019.

29 CFR 1926.405(b)(2):   In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:  a)  1706 Paterson Plank Road,  5th Floor, North Bergen, NJ.  General Contractor and Subcontractor Employees and other contractors on this multiemployer site were exposed to electric shock as the energized branch circuits at the site were missing their faceplates.                                                      Violation observed on or about 12/10/2019.

29 CFR 1926.501(b)(14):   Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.   a)  1706 Paterson Plank Road,  4th Floor, North Side, North Bergen, NJ.  Sheetrock subcontractor had removed 2 x 4 guardrails from the wall opening to the elevator shaft exposing other contractors on the site to falls of 4 stories to the bottom of the shaft.                                                 Violation observed on or about 12/10/2019.  b)  1706 Paterson Plank Road, 2nd Floor, West Side, Approximately 20 Window Openings, North Bergen, NJ.  Site employees were exposed to 15 foot falls to the outside ground below from approximately 20 wall openings where workers could walk directly from the main floor level right out the unguarded openings.                                                    Violation observed on or about 12/10/2019.
