29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:    On or about 12/10/2019 and at times prior, at 16660 Sunset Drive, Lot # 64B, Botaniko worksite, Weston, Florida, employees were exposed to falling approximately 12 feet to ground level and being struck by falling items such as, but not limited to, scaffolding parts, when working from a scaffold that had missing cross braces.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about 12/10/2019 and at times prior, at 16660 Sunset Drive, Botaniko, Weston, Florida, employees were not trained to recognized and avoid unsafe conditions such as fall hazards associated with scaffolding, when working from a scaffold that was lacking mud sills, access ladder cross braces, and base plates, thereby exposing employees to fall and struck-by hazards.

Construction BCA LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1247445, citation number 01, item number 01 and was affirmed as a final order on 04/06/2018, with respect to a workplace located at 16479 Botaniko South, Lot 57B, Weston, Florida 33326.

Crossbraces were used as a means of access:    On or about 12/10/2019 and at times prior, at 16660 Sunset Drive, Lot 64B, at Botaniko, Weston,Florida, a ladder was not provided and employees were climbing the crossbraces of a fabricated frame scaffold, thereby exposing the workers to a fall hazard of 12 feet to ground level.
