29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Laser Station 7-8 - On or about August 27, 2019, the employer did not ensure that machine guarding was provided to prevent employees from entering the railing system where the STOPA cart travels. The STOPA cart control button was held down with tape allowing the cart to travel automatically and an employee inside the railing system was struck by the cart causing a leg injury.            No abatement certification or documentation is required for this item.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    (a) Facility Wide - On December 11, 2019, the employer did not provide a medical evaluation to employees required to use an 3M6503 half face tight-fitting negative pressure respirator when cleaning inside the laser booth.       In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(m)(2)(i): The employer did not establish a record of the qualitative and quantitative fit tests administered to an employee which included the information required by 29 CFR 1910.134(m)(2)(i)(A) through (m)(2)(i)(E):  (a) Facility Wide - On December 11, 2019,the employer did not maintain fit test records for all employees required to wear tight fitting respirators.    In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
