29 CFR 1910.140(c)(13)(i): Anchorages except window cleaners' belt anchors covered by paragraph (e) of this section, was not capable of supporting at least 5,000 pounds (22.2 kN) for each employee attached; or (c)(3)(ii) was not designed, installed, and used under the supervision of qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two: (a) On or about December 02, 2019 - Plant 3, Freight Room/Storage, employees using personal fall arrest systems were not provided with an approved anchor when removing racks and rack shelves. Employees were anchoring to a scissor lift while outside of the scissor lift which was not approved by the manufacturer, anchoring to racks that had not been approved for anchors, or not anchoring when leaving the scissor lift. Roof beams/joists were not located over all work areas.

29 CFR 1910.30(c)(2): The employer did not retrain employees when changes in the types of fall protection systems or equipment used render previous training obsolete or inadequate: (a) On or about December 09, 2019 - Plant 3, Freight Room/Storage, employees were exposed to fall hazards of four feet and higher when they had not been trained to know what type of fall protection to use when they could not reach the screws holding down the sheet metal decking on the back side of racks due to the back-to-back racks.

29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made and permanent aisles or passageways were not appropriately marked: (a) On or about December 09, 2019 - Plant 3, Freight Room/Storage, employees were exposed to struck-by hazards when there were only a few inches to spare between the forklift and the racks when turning into the aisles due to pallets of wrapped product stored on the floor blocking access to the racks. There were no aisles marked in the area where pallets were placed on the floor.
