29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 part per million:    On or about December 3, 2019, in the assembly area, an employee was exposed to carbon monoxide concentrations at an eight-hour time weighted average which exceeded the OSHA PEL of 50 pat1s per million.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:   On or about December 3, 2019, the employer did not evaluate the respiratory hazards for employees exposed to hazards associated with forklift exhaust fumes and heater exhaust fumes such as but not limited to headaches, nausea, dizziness and confusion from exposure to chemicals such as carbon monoxide when employees are producing, assembling, packing and shipping parts in areas of the facility with natural gas powered radiant heaters and forklift traffic.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    On or about December 3, 2019, the employer had not trained employees exposed to hazardous chemicals such as, but not limited to carbon monoxide which is contained in forklift and heater exhausts on the health hazards associated with the chemical, methods to detect release of a hazardous chemical and measures employees can take to protect themselves from exposure to the chemicals.
