29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.:    On or about December 12, 2019, the employer failed to ensure that powered industrial vehicles (Hyster, serial #D187V30051A and Hyster, serial #F10BVf3131V) were removed from service when a defective, such as but not limited to working lights and horns, were found.  Both of the Hyster PIV's were in service where one had a non working horn and the other non working lights.

29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.  On or about December 12, 2019, the employer failed to ensure that an emergency exit door located in the mold room was unlocked so that employees could evacuate safely and in an unobstructed way in the event of an emergency.

These facilities were kept in disarray with trash around and in the sinks and a large amount of filth built up all throughout.

29 CFR 1910.141(d)(2)(ii): Each lavatory shall be provided with hot and cold running water, or tepid running water:  On or about December 12, 2019, the employer failed to ensure that the toilet/washroom facilities located in the front office of the plant had hot or tepid running water.  Both the men's and women's facilities had no hot or tepid water.
