Main Street, Yardley, PA, Worksite, Roof - On or about December 12, 2019, employees performing roofing work on a steeply pitched roof of a residence, approximately 20 feet above grade, were not protected from falling by any means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 70 S. Main Street, Yardley, PA, Worksite - On or about December 12, 2019, employees performing roofing work on a steeply pitched roof of a residence were using an aluminum extension ladder to access the work area that did not extend 3 feet above the landing surface.
