29 CFR  1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a) Jobsite located at 260 Powell Rd, Springfield, PA 19064 - On or about July 12, 2023 the employer did not initiate or maintain an accident prevention program to address the associated hazards that employees are exposed to during roofing activities, such as but not limited to fall hazards.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite located at 260 Powell Rd, Springfield, PA 19064 - On or about July 12, 2023, the employer did not provide a training program for each employee conducting roofing operations from the roof of a two-story residential building, and exposed to fall hazards of approximately 20 feet in the recognition and avoidance of fall hazards.

29 CFR  1926.416(a)(1):The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work.  a) Jobsite located at 260 Powell Rd, Springfield, PA 19064 - The employer exposed employees installing a new roof on a residential home to electrical hazards by permitting them to work in proximity to a service drop line without ensuring its circuit was effectively guarded. 29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b):  a) Jobsite located at 260 Powell Rd, Springfield, PA 19064 - On or about July 12, 2023 the employer did not ensure employees conducting roofing operations from the roof of a two story residential building, approximately 20 feet above the ground level, were protected from falls by the use of a fall protection system.  CH Home Remodeling LLC. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1451414, citation number 1, item number 1, and was affirmed as a final order on 4/27/2020, with respect to a workplace located at 70 S. Main Street, Yardley, PA 19067.
