29 CFR 1910.178(m)(8): There shall be sufficient headroom under overhead installations, lights, pipes, sprinkler system, etc: (a) On or about December 9, 2019, the employer did not ensure the produce containers being unloaded from a box truck using a Toyota Pallet Jack (Model 8HBE30 & Serial No. 8HBE30-49811), maintained sufficient room under the overhead door of the box truck. This exposed the pallet jack operator to a crushing hazard created by the overhead door and load on the pallet jack.
