The employer failed to implement the hearing conservation program to include monitoring to identify employees for inclusion into the program, audiometric testing to provide surveillance for noise-induced hearing loss (NIHL), and hearing conservation training.  (a) On June 4, 2025, an employee working on the potato line was exposed to noise at 109.1% of the permissible daily dose, or an average sound level of 91.1 dBA, as measured over 448 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure of 90.6 dBA.  (b) On June 4, 2025, an employee working in the edible cuts area was exposed to noise at 87.3% of the permissible daily dose, or an average sound level of 89.5 dBA, as measured over 446 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure of 89 dBA.  (c) On June 4, 2025, an employee working in the edible cuts area was exposed to noise at 55.4% of the permissible daily dose, or an average sound level 85.7 dBA, as measured over 447 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure of 85.7 dBA.
