LOUISVILLE self-supporting ladder to accessing a beam of the gazebo approximately 11 ft. above the ground; on or about 07/02/2019.     Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) Worksite, 111 Pheasant Ln, Southampton, New York - An employee was walking and working along the beam of the gazebo approximately 11 ft. above the ground and was not protected from falling; on or about 07/02/2019.     Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.        Troy Construction & Design, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1150143, Citation Number 1, and Item Number 2 and was affirmed as a final order on 08/08/2016, with respect to a workplace located at 28 Dune Rd, Quogue, New York.
