29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a) Grinder Pit: A 33-inch wide open-sided edge at the grinder pit exposed employees to a fall of about 5 1/2 feet, on or about December 18, 2019.  b) Grinder Pit: A 37-inch wide open-sided edge at the ladder access for the grinder pit exposed workers to a fall of about 5 1/2 feet, on or about December 18, 2019.  b) Mezzanine: Missing sections of guard railing on the mezzanine exposed workers to a fall of about 13 feet, on or about December 18, 2019.  c) Mezzanine Stairs: A 20-inch open-sided edge adjacent to the stair landing exposed employees to a fall of about 13 feet, on or about December 18, 2019. 29 CFR 1910.29(b)(1): The top edge height of top rails, or equivalent guardrail system members, were not 42 inches, plus or minus 3 inches, above the walking-working surface:  a) Grinder Pit: Pallets placed next to the grinder pit railing for employees to stand on during grinder operations reduced the height of the rail to between 33 and 37 inches, which exposed employees to the possibility of falling over the rail, on or about December 18, 2019. 29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  a) Production Area: Proper protective clothing and goggles were not provided for an employee exposed to sulfuric acid while inspecting and servicing a forklift battery, on or about February 4, 2020.  b) Large grinder, the abdomens, waists and legs of employees who were grinding the 55 gallon poly drums were exposed to liquids, including caustic food sanitizers, that were left inside the flattened drums, and did not wear chemical resistant clothing or full sized aprons, on or about February 4, 2020.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Production Area: An eyewash was not provided for an employee who services forklift batteries containing corrosive sulphuric acid, on or about February 4, 2020.  b) Large grinder, employees were exposed to potential eye hazards from contact with liquid chemicals while shredding baled and compressed drums that contained caustic and corrosive residual liquids, and an eyewash was not present in the immediate area, on or about, February 4, 2020.  ABATEMENT CERTIFICATION REQUIRED
