29 CFR 1910.28(b)(2)(i): The employer did not ensure employees in a hoist area were protected from falling 4 feet or more to a lower level. a) All American Pharmaceutical, Blending and Pre-Mix Department: On or about December 18, 2019, and at times prior, employees were exposed to fall hazards of approximately 6 feet while utilizing walking working platforms without a mid-rail to access Machine 5. The employer failed to implement a complete fall protection system during these operations. b) All American Pharmaceutical, Blending and Pre-Mix Department: On or about December 18, 2019, and at times prior, employees were exposed to fall hazards of approximately 6 feet while utilizing walking working platforms without a mid-rail to access the Blender. The employer failed to implement a complete fall protection system during these operations. c) All American Pharmaceutical, Blending and Pre-Mix Department: On or about December 18, 2019, and at times prior, employees were exposed to fall hazards of approximately 7 feet 5 inches while utilizing walking working platforms without a mid-rail to access the Mixer. The employer failed to implement a complete fall protection system during these operations.
