29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a)  On or about December 4, 2019, employees were exposed to head injuries because they were not wearing protective helmets when they were working under other employees performing roofing activities above who were throwing down debris to the employees on the ground so that it could be placed in trash receptacles.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, is not being protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    (a) Drury Brothers Roofing, Inc., is failing to ensure that its employees are protected from fall hazards during roofing work.  This was most recently observed on December 4, 2019, when employees of Drury Brothers Roofing Inc. were reroofing a commercial building without fall protection.  The eaves of the roof were 10 feet above the ground with a 4:12 pitch roof.     Drury Brothers Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1405617, citation number 1, item number 1, and was affirmed as a final order on August 22, 2019, with respect to a workplace located at 202 Manitou Ave., Manitou Springs, CO.    Drury Brothers Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1251645, citation number 1, item number 1, and was affirmed as a final order on August 30, 2017, with respect to a workplace located at 602 Main Street in Ordway, Colorado.    Drury Brothers Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1202462, citation number 1, item number 1, and was affirmed as a final order on February 14, 2017, with respect to a workplace located at 4346 Yellow Dock Point in Colorado Springs, Colorado.
