29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded:  a) Jose Gonzalez Construction located at 230 E. Long Street, Columbus, OH 43215: On and preceding December 10, 2019, the employer did not ensure that vertical lifelines used for fall protection at or near leading edges were protected against being cut.  A vertical lifeline, rope grab was used as a personal fall arrest system (PFAS) by the framing crews near 90 degree leading edges while installing parapet walls and wood decking at heights beyond 6 feet.  The lifeline was not protected from being cut by the 90 degree edge in the event of a fall.  The following condition exposed the employee to falls up to 70 feet.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a) Jose Gonzalez Construction located at 230 E. Long Street, Columbus, OH 43215: On and preceding December 10, 2019,  the employer did not ensure that vertical lifelines used for fall protection were rigged so that employees could not free fall more than 6 feet nor contact any lower level.  A vertical lifeline, rope grab was used as a personal fall arrest system (PFAS) by the framing crews near 90 degree leading edges while installing parapet walls and wood decking at heights beyond 6 feet.  Lifelines were not rigged in a way to limit fall distance to 6 feet and or keep the worker from contacting the lower level.  The following condition exposed the employee to falls exceeding 70 feet.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the correct use and operation of personal fall arrest equipment.  a) Jose Gonzalez Construction located at 230 E. Long Street, Columbus, OH 43215: On and preceding December 10, 2019, the employer did not ensure that employees were trained in the correct use personal fall arrest equipment at the job site.  A vertical lifeline, rope grab was used as a personal fall arrest system (PFAS) by framing crews near 90 degree leading edges while installing parapet walls and wood decking at heights beyond 6 feet.  The lifeline system being used was not designed for use as a PFAS near leading edges. The following condition exposed the employee to falls exceeding 70 feet.
