29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.  a) On December 23, 2019, employees were exposed to fall hazards at 10 Westlake Dr. South Barrington, IL 60010 while performing rood work. Employees were working on a platform approximately 12 feet in length with each end of the platform extending over its support more than 18 inches.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On December 23, 2019, employees were exposed to fall hazards at 10 Westlake Dr. South Barrington, IL 60010 while performing rood work. Employees were working more than 6 feet above the ground without the use of guardrail systems, safety net systems, or personal fall arrest systems while working on a ladder jack scaffold.   No abatement documentation is required for this citation as it was corrected during inspection.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) On December 23, 2019, employees were exposed to fall hazards at 10 Westlake Dr. South Barrington, IL 60010 while performing rood work. Employees were working more than 6 feet above the ground without the use of guardrail systems, safety net systems, or personal fall arrest systems.   No abatement documentation is required for this citation as it was corrected during inspection.
