(b) Power House - 6th floor - Bay 12 - On or about January 6, 2020 and at times prior thereto, employees were exposed to fire and smoke inhalation hazards in that a portable fire extinguisher was more than approximately 114'' of travel distance.

29 CFR 1910.164(c)(4): The employer shall assure that the servicing, maintenance and testing of fire detection systems, including cleaning and necessary sensitivity adjustments are performed by a trained person knowledgeable in the operations and functions of the system. (a) Powerhouse - Control Room - On or about January 6, 2019 and at times prior thereto, the servicing, maintenance and testing of the Very Early Smoke Detection (VESDA) system was not performed by a trained person knowledgeable in the operations and function of the system. This exposed employees to a system that may not work properly, which could result in fire and smoke inhalation. (b) Powerhouse - Control Room - On or about January 6, 2019 and at times prior thereto, the servicing, maintenance and testing of the GE ESTxxx, which controls the elevators, was not performed by a trained person knowledgeable in the operations of the system. (c) Powerhouse - 3rd floor - On or about January 6, 2019 and at times prior thereto, the servicing, maintenance and testing of the Fine Water Mist system.
