(a) Powerhouse - 8th floor (phase gallery) - On or about January 6, 2020 and at times prior thereto, the exit routes throughout the 8th floor including, but not limited to: bay 9 through 16 were not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (b) Powerhouse - 7th floor (cable gallery) - On or about January 6, 2020 and at times prior thereto, the exit routes throughout the 7th floor including, but not limited to: bay 9 through 16 were not clearly visible, this exposed employees to fire and smoke inhalation hazards. 29 CFR 1910.157(d)(2): Portable fire extinguishers for use on Class A fires were not distributed so that the travel distance for employees to any extinguisher was 75 feet or less.  (a) Power House - 6th floor - Bay 16 - On or about January 6, 2020 and at times prior thereto, employees were exposed to fire and smoke inhalation hazards in that a portable fire extinguisher was approximately 125' of travel distance.  (b) Power House - 6th floor - Bay 12 - On or about January 6, 2020 and at times prior thereto, employees were exposed to fire and smoke inhalation hazards in that a portable fire extinguisher was more than approximately 114'' of travel distance.

29 CFR 1910.164(c)(1): The employer did not maintain all systems in an operable condition except during repairs or maintenance.  (a) Powerhouse - Control Room - On or about January 6, 2019 and at times prior thereto, the Very Early Detection (VESDA) System was not maintained in a operable condition in that the Siemens software was expired and no longer working. This prevented immediate identification of the location of alarms. Employees were exposed to fire and inhalation hazards.

29 CFR 1910.164(c)(4): The employer shall assure that the servicing, maintenance and testing of fire detection systems, including cleaning and necessary sensitivity adjustments are performed by a trained person knowledgeable in the operations and functions of the system.  (a) Powerhouse - Control Room - On or about January 6, 2019 and at times prior thereto, the servicing, maintenance and testing of the Very Early Smoke Detection (VESDA) system was not performed by a trained person knowledgeable in the operations and function of the system. This exposed employees to a system that may not work properly, which could result in fire and smoke inhalation.  (b) Powerhouse - Control Room - On or about January 6, 2019 and at times prior thereto, the servicing, maintenance and testing of the GE ESTxxx, which controls the elevators, was not performed by a trained person knowledgeable in the operations of the system.  (c) Powerhouse - 3rd floor - On or about January 6, 2019 and at times prior thereto, the servicing, maintenance and testing of the Fine Water Mist system.

(a) The Dalles Dam - Powerhouse - On or about January 6, 2020 and at times prior thereto, employees were exposed to fire and smoke inhalation in that employees are working in remote locations, throughout the power house, without adequate emergency notification.

(a) Powerhouse - 5th floor - XJ Circuit Breaker Panel - On or about January 6, 2019 and at times prior thereto, an employee was exposed to contact with 125 Volt Direct Current (VDC) while applying a lock and tag in the circuit breaker panel.
