29 CFR 1910.22(c): Access and egress. The employer must provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces. On January 2, 2020, employees were exposed to falls while accessing and egressing a roof surface that was 3.8' above the lower surface when a bucket was used in place of a ladder.

29 CFR 1910.28(b)(13)(i): Work on low-slope roofs. When work is performed less than 6 feet (1.6 m) from the roof edge, the employer must ensure each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system. On January 2, 2020, an employee was working at the edge of the roof patching the membrane around a roof drain when his harness and self-retracting lifeline were not connected to an anchor point.
