29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a)  The employer failed to provide documentation to the Agency demonstrating that abatement is completed for the serious violations that were issued by the Agency.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   (a)  On or about January 6, 2020, at the construction site located at N120W12431 Freistadt Rd, Germantown, Wisconsin, employees were performing roofing work on a two story residential home with a pitch of approximately 8 in 12 and up to approximately 10 8/10 feet in height with no fall protection, thereby exposing the employees to a fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about January 6, 2020, at the construction site located at N120W12431 Freistadt Rd, Germantown, Wisconsin, employees were exposed to a fall hazard of approximately 10' 8/10" and had not been trained to recognize and minimize the fall hazards associated with their work.
