The forklift operators had not received specific training for the various model forklifts.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On or about 01/06/2020, MERS/Goodwill, located at 3728 Market Street, St. Louis, MO, employee(s) were exposed to a forklift in need of repair. The forklift in need of repair was a Raymond clamp truck, Model: 470-C50HM, S/N: 470-08-HM20048. The manufacturers yellow flashing warning light was not functioning. The forklift needed to be taken out of service until this warning system was repaired.
