On or about January 7, 2020, at the above addressed jobsite, an employee working near heavy equipment installing roof trusses, was not wearing a hard hat to protect against possible head injuries from impact of equipment and materials.

On or about January 7, 2020 at the above addressed jobsite, employees using pneumatic and cordless nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval.  a.  On or about January 7, 2020, at the above addressed jobsite, the employer allowed his employees to operate and lift roof truss panel sections using  lifting straps placed around the left fork on the United Rentals Rough Terrain Forklift number 10072474, when this practice is not recommended and or approved by the manufacturer. The employees were thereby exposed to struck by hazards due to the possibility of the failure to the fork.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a.  On or about January 7, 2020, at the above addressed jobsite, employees performing sheeting operations were exposed to falls ten (10) feet, two (2) inches above the ground, created by the presence of unprotected sides.  b.  On or about January 7, 2020, at the above addressed jobsite, employees performing carpentry operations from the roof were exposed to falls, of thirteen (13) feet four (4) inches, created by the presence of unprotected sides.  K & J Builders LLC was previously cited  for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1226345, citation number 1, item number 1 and was affirmed as a final order on July 13, 2017, with respect to a workplace located at 100 Harvester in Burr Ridge, IL.  K & J Builders LLC was previously cited  for a  violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1059605, citation number 1, item number 1b and was affirmed as a final order on July 15, 2015, with respect to a workplace located at 9749 Clark Dr. in Rossford, OH.
