29 CFR 1910.23(b)(2): Ladder rungs, steps, and cleats are spaced not less than 10 inches (25 cm) and not more than 14 inches (36 cm) apart, as measured between the centerlines of the rungs, cleats, and steps, except that: (a) On or about January 9, 2020, the employer did not ensure ladder rungs on Press 38 were spaced not less than 10 inches apart. The ladder rungs were spaced 8 inches apart, exposing employees to a fall hazard of approximately 8 feet.

29 CFR 1910.29(b)(2): The employer did not ensure that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high: (a) On or about January 9, 2020, the employer did not ensure service platform on Press 38 was equipped with a mid-rail. This exposed employees to a fall hazard of approximately eight (8) feet. (b) On or about January 9, 2020, the employer did not ensure service platform on Press 19 was equipped with a mid-rail. This exposed employees to a fall hazard of approximately eight (8) feet.
