29 CFR 1910.22(a)(2): The floor of a workroom was not maintained in a clean condition. a) On or about January 07, 2020 in the CNC work area and along the wall, the employees were working in the warehouse with a build-up of combustible saw dust throughout the facility.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. a) On or about January 07, 2020 on the east side of the warehouse a Duracraft Industrial Machine Drill Press (Model: 41617) was being utilized with inadequate strain relief on an energized power cord covered with saw dust.
