Deficiencies noted during the inspection included, but were not limited to, failing to provide appropriate cave in protection for a trench determined to be at least 7 1/2 feet in depth, not controlling water accumulation, allowing piping to remain at the edge of the trench while someone worked in it and not ensuring that a safe means of egress was provided in the trench.
