29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions or an inspection was not conducted by the competent person prior to the start of work and as needed throughout the shift or after every rainstorm or other hazard increasing occurrence:   a) Job site, at/near 936 Sheraton Drive, Mars, PA: On or about January 8, 2020, during sewer line installation work, the employer did not ensure that a competent person inspected the trench prior to working in it. Deficiencies noted during the inspection included, but were not limited to, failing to provide appropriate cave in protection for a trench determined to be at least 7 1/2 feet in depth, not controlling water accumulation, allowing piping to remain at the edge of the trench while someone worked in it and not ensuring that a safe means of egress was provided in the trench.

29 CFR 1926.20(b)(2): The employer did not ensure that a program of frequent and regular inspections of the job sites, materials, and equipment were made by competent persons:   Instances including, but not limited to, the following:  a) Job site, at/near 936 Sheraton Drive, Mars, PA: On or about January 15, 2020, during construction activities work, the employer did not ensure that an appropriate system of hazard identification and corrective action was implemented at the site. Deficiencies noted during the inspection included, but were not limited to, subcontractors kneeling on the top of step ladders and electrical hazards resulting from power conductors being exposed to damage.
