29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    2 Boland Way, Springfield, MA: On or about January 13, 2020the employer had not instructed each employee in the recognition and avoidance of unsafe conditions and the regulations to his/her environment to control or eliminate exposure to hazards such as but not limited electrical hazards, demolition hazards, and  respiratory hazards while performing demolition activities in the ballroom  rehabilitation construction project.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.405(j)(1)(ii): Fixtures, lampholders, rosettes, or receptacles were not securely supported:    2 Boland Way, Springfield, MA: On or about January 13, 2020 employees were observed working in a demolition area where the fixtures and lamp holders were supported by the electrical wires and conduit supplying them and not by the means designed by the manufacturer such as but not limited to housing screw attachments and support wires.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.850(d): When it was necessary to maintain power, water or other utilities during demolition, such lines were not temporarily relocated, as necessary, and protected.    2 Boland Way, Springfield, MA: On or about January 13, 2020 employees were observed working in a demolition area where it was necessary to maintain power and such lines were not relocated as necessary or adequately supported exposing employees to caught by/struck by hazards and electrical shocks and burns.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
