OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees; in that, employees were exposed to a fall hazard while operating an industrial scissors lift:   At 3400 N. Federal Hwy, Fort Lauderdale, Florida, 33406: On or about January 03, 2020 and at times prior, employees were exposed to a fall hazard of 52 inches to ground level, when unloading a tractor-trailer with an industrial scissors lift that was operated from its platform, with the following conditions:  i) did not receive prior written approval from its' manufacturer, for the installation of an access gate system  ii) had a rusted automatic closing and locking mechanism on one-half of the access gate system;   iii) did not have access gates or personal restraint upper and lower chains on one end of the scissors lift;   iv) did not have toe boards on both sides of the guardrails systems;   v) did not have a manufacturer's  name plate;  vi) did not have an Operators/Maintenance Manual for the equipment from the manufacturer; and  vii) did not have warning labels.
