29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry: a.) Rdg Group, Inc.; Bfg Manufacturing Services, Inc.; Punxsutawney Finishing Works, Inc., production areas throughout the facility, on or about January 14th, 2020, and times prior thereto: Employees periodically entered and performed work in permit required confined spaces located in the facility which included, but were not limited to, tanks on line 32, and tanks on line 22. The company's written permit required confined space program did not describe potential rescue procedures in the event of an emergency within the spaces. Additionally, employees were not aware of what procedures to follow in the event a rescue was needed. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition: a.) Rdg Group, Inc.; Bfg Manufacturing Services, Inc.; Punxsutawney Finishing Works, Inc., Area next to parts line 22, on or about January 14th, 2020, and times prior thereto: Employees were utilizing compressed air guns/nozzles (Guard Air brand Long John Safety air gun 75LJ) to clean/dry off metal parts that were saturated with water/debris. The company did not ensure the safe condition of the compressed air gun/nozzle in that, the nozzle was damaged/broken and no longer contained relief venting. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Rdg Group, Inc.; Bfg Manufacturing Services, Inc.; Punxsutawney Finishing Works, Inc., Area next to parts line 22, on or about January 14th, 2020, and times prior thereto: Employees were utilizing compressed air guns/nozzles (Guard Air brand Long John Safety air gun 75LJ) to clean/dry off metal parts that were saturated with water/debris. Measurements of compressed air next to line 22 were measured at approximately 35 p.s.i. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Rdg Group, Inc.; Bfg Manufacturing Services, Inc.; Punxsutawney Finishing Works, Inc., production areas, on or about January 14th, 2020 and times prior thereto: An employee working in areas including, but not limited to, line 41 area of the facility experienced a standard threshold shift in hearing. The employer did not inform the employee of the standard threshold shift, in writing, within 21 days.
