29 CFR 1910.95(g)(6): "Annual audiogram." At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels as required in 29 CFR 1910.95(g)(6). On or about February 3, 2020, employees were not adequately protected from noise hazards by the failure to use audiometric testing on an schedule to identify standard threshold shifts that may be indicative of noise-induced hearing loss. (a) The employee trimming finished pieces in area one (1) was exposed to noise at 85.0 decibels (50.0 percent dose) measured on the A scale, equivalent to the time-weighted average threshold for audiometric testing of 85 decibels as measured on the A scale. The sample was taken on February 3, 2020, during a 500 minute sampling period. (b) The employee operating the die cast machine in area five (5) was exposed to noise at 88.3 decibels (79.2 percent dose) measured on the A scale, exceeding the time-weighted average threshold for audiometric testing of 85 decibels as measured on the A scale. The sample was taken on February 3, 2020, during a 489 minute sampling period. (c) The employee operating the vibratory deburring machine by area 12 was exposed to noise at 89.0 decibels (87.62 percent dose) measured on the A scale, exceeding the time-weighted average threshold for audiometric testing of 85 decibels as measured on the A scale. The sample was taken on February 3, 2020, during a 490 minute sampling period.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required to wear PPE as necessitated by this standard, including but not limited to the following sections: - When PPE is necessary; - What PPE is necessary; - How to properly don, doff, adjust, and wear PPE; - The limitations of PPE; - The proper care, maintenance, useful life and disposal of PPE. (a) On or about January 17, 2020, employees were observed working on or within the vicinity of an operational vibratory deburring machine without utilizing hearing protection, exposing employees to noise-induced hearing loss. Employees were not adequately trained to protect themselves from noise-induced hearing loss by using hearing protection as required by the company personal protective equipment (PPE) hazard assessment.
