29 CFR 1926.451(a)(1): Each scaffold and scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.  On January 10, 2010 on the fourth floor south tower: A scaffold loaded with granite reinforced concrete (GRC) collapsed while three employees used the scaffold to transfer door beams up to the next level. Employees were exposed to fall hazards.

29 CFR 1926.451(a)(3): Each suspension rope, including connecting hardware, used on non-adjustable suspension scaffolds was not capable of supporting, without failure, at least 6 times the maximum intended load applied or transmitted to that rope  On January 10, 2010 on the fourth floor south tower: Wire ropes used to suspend the scaffold broke while three employees used the scaffold to transfer equipment to the next level. Employees were exposed to fall hazards.

On January 10, 2010 on the fourth floor south tower: The needle beam type scaffolding was not designed to hold the weight of the scaffolding, added materials and workers. A scaffold loaded with granite reinforced concrete (GRC) collapsed while three employees used the scaffold to transfer door beams up to the next level. Employees were exposed to fall hazards.

29 CFR 1926.451(d)(12)(vi): When U-bolt clips are used, the U-bolt shall be placed over the dead end of the rope, and the saddle shall be placed over the live end of the rope.  January 10, 2010 on the fourth floor south tower: The U section of U-bolt wire rope clips used as part of the system to support the scaffolding were not installed in contact with the dead end and the saddle on the live end of the rope. Employees were exposed to fall hazards.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  On January 10, 2010 on the fourth floor south tower: The scaffolding in use to transfer materials was not inspected by a competent person before the start of work. Employees were exposed to fall hazards.
