29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:    RD Quality Construction, LLC- On or about January 22, 2020, at the address of 14530 Dexter Falls Road, Perrysburg, OH: The employer did not have a plan or program to identify jobsite hazards which assists employees in maintaining a safe working environment.  Employees who performed roofing activities were exposed to fall hazards that would result in injuries including, but not limited to, bone fractures, other serious injuries or death.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    RD Quality Construction, LLC - On or about January 22, 2020, at the address of 14530 Dexter Falls Road, Perrysburg, OH: The employer did not ensure that a fall protection training program was in place for employees that are exposed to fall hazards over six feet in residential roofing.  Employees who performed roofing activities were exposed to fall hazards that would result in injuries including, but not limited to, bone fractures, other serious injuries or death.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    RD Quality Construction, LLC - at the address of 14530 Dexter Falls Road, Perrysburg, OH: The employer did not ensure that the employee who operated the nail gun was wearing proper eye protection while performing roofing activities.   The employee who operated the nail gun was exposed to flying object hazards that would cause injuries including, but not limited to, temporary vision loss or scratches to the eye.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    RD Quality Construction, LLC- On or about January 22, 2020, at the address of 14530 Dexter Falls Road, Perrysburg, OH: The employer did not ensure that employees were protected from fall hazards while performing roofing activities without the benefit of fall protection.  Employees were exposed to falls of approximately 9 feet or greater which could result in injuries that include, but are not limited to, bone fractures, other serious injuries, or death.
