29 CFR  1926.451(b)(7):On scaffolds where platforms are overlapped to create a long platform, the overlap was not at least 12 inches (30 cm) and the platforms were not nailed together or otherwise restrained to prevent movement:  a)	RD Quality Construction, LLC. On or about October 4, 2022 at the address of 19654 Kellogg Rd. Bowling Green OH 43402: The employer did not maintained the 12 inches (30 cm) requirement for overlapping scaffold planks nor were the planks nailed together or restrained to prevent movements. Employees utilizing scaffold to performing framing work activities were exposed to fall hazards from a fall of 20 feet or greater to the ground below.

29 CFR  1926.451(c)(2)(v):Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  a)	RD Quality Construction, LLC. On or about October 4, 2022 at the address of 19654 Kellogg Rd. Bowling Green OH 43402: The employer did not ensure that the aluminum job box scaffold platform used to hoist employees to higher levels was entirely attached to the forks of the rough terrain forklift model #644E-42 serial #RPS T8424W1547. Employees using Scaffold in this condition as well as others working in and around forklift operating areas was exposed to struck-by injuries, fall injuries, tipping, and rollover incidents.

Employee(s) operating in this condition were exposed to fall hazards from a fall of 20 feet or greater to the ground below.
