29 CFR 1926.454(c): When the employer had reason to believe that an employee lacked the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, the employer did not retrain each such employee so that the requisite proficiency is regained: a) Job site, near 4431 William Penn Highway, Murrysville, PA: On or about January 14, 2020, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were in use while an employee was working from an aerial lift basket at a height of approximately 12 feet in that the employee was using inappropriate fall protection devices which would not have protected him from hitting the ground in the event that he was thrown from the lift.
