29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 23, 2020 the employer failed to ensure that the CNC vertical milling machine (max-4xp, serial number 701094) was effectively guarded to prevent employees from reaching into the machine while in operation. The front door of the machine is able to be opened during production or not interlocked to stop the operation in the event it is opened. The side of the machine contained a plexiglass window which was broken and missing a section. Bula Forge & Machine Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number , citation number 1, item number 6 and was affirmed as a final order on February 16, 2017 with respect to a workplace located at 3001 West 121st Street, Cleveland, OH.
