29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 21, 2026, at 3001 West 121st, Cleveland, OH 44111, the employer failed to ensure employees feeding the furnace were protected from caught in/crushing hazards in that the foot pedal used to feed billets into the furnace was close enough where employees could place their hands in the point of operation.
