Site - On or about January 21, 2020, employees engaged in framing activities were exposed to fall hazards of over 20 feet, where fall protection was not provided or utilized to prevent the hazard. Instance b. Site - On or about January 21, 2020, employees engaged in framing activities were exposed to fall hazards of over 9 feet, where the three staircase opening were not covered or guarded to prevent employees fall hazards.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76cm) whichever is less were not equipped with one stairrail system along each unprotected side or edge: Site - On or about January 21, 2020, the three interior staircases utilized by employees as means of accessing the second story work area were not provided with a stair rail on the open side of the staircases to prevent employees' fall hazards.
