29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a. Site - On or about January 23, 2020, the 10 foot stepladder utilized by employees to access the third floor level was not adequate, in that the ladder did not extend 3 feet above the upper landing surface.
