OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushed-by hazard:    On or about January 21, 2020, southwest side of a church parking lot, employee unloading a Platform Basket (Backyard Lift style) aerial lift, off a trailer with right wheels in ditch next to the driveway, was exposed to the hazard of crush-by when extending the tracks while in fall shadow.      ABATEMENT: Among other methods, feasible and acceptable abatement methods include:     1.  Improved training on loading and unloading, including to        a.  Ensure the aerial lift is stable on the trailer before attempting to move/unload.        b.  Operate the aerial lift during loading/unloading from a distance outside the fall shadow of the lift.    2.  Development and implementation of JHAs specific to loading and unloading aerial lifts    3.  Improve vetting of new types of equipment for safety hazards.
