On or about 1/17/20, an energy control procedure for machine BM-4 did not identify the process for shutting down, isolating, blocking, or securing of pneumatically controlled components of the equipment.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Manufacturing floor, 256 Andover Street, Wilmington, MA. On or about 1/17/20, an employee operating BM 4, a vertical Bridgeport mill, was contacted by the point of operation from an unintended activation of the spindle piston. The machine was not outfitted with controls or guarding that would prevent the operator from contacting the point of operation or being contacted by the point of operation.
