29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: On or about January 30, 2020, during the installation of new waste water lines in an open trench at the work site located at 7600 Block Wales Ave NW, North Canton, Ohio 44720, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to cave in hazards (trench with near vertical walls with a varying depth of 6.2 to 7.3 feet) and struck by hazards (employees working in a manhole where concrete was being poured and splashed in all direction) from working.
