29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a.) Unique Fabrications, Inc. 19059 Paint Blvd, Shippenville, PA 16254-Punch Press Area/Front shop area-On or about February 6th, 2020 and times prior thereto: An employee in the punch press/front shop area was provided with a baseline audiogram on May 4th, 2016, the employee was not provided with annual audiograms on or around May 2017, May 2018, and May 2019. The employee performing operations including, but not limited to, grinding of metal parts, punch press operations of metal parts was exposed to noise at a level of 116.4 % of the allowable limit during a 410 minute sampling period on February 6th, 2020; exposure calculations included a zero increment for the 70 minutes not sampled. This exposure is equivalent to a sound level of 91.1 dBA for the dose and time of the employee exposure.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields: a.)Unique Fabrications, Inc. 19059 Paint Blvd, Shippenville, PA 16254-Fabrication/Back shop area (Northwest wall), On or about January 30th, 2020 and times prior thereto: Employees working near or around MIG welding operations were not protected from the welding rays, in that, the employees were not protected by welding shields/screens during travel from the front shop area to the back shop area. Additionally, employees were not protected from welding rays while performing work operations at the Piranha iron worker in the back shop area. The Unique Fabrications, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.252(b)(2)(iii) which was contained in OSHA inspection number 1136188, citation number 1, item number 5 and was affirmed as a final order on May 16th, 2016 , with respect to a workplace located at 19059 Paint Blvd, Shippenville, PA 16254 . ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: a.) Unique Fabrications, Inc. 19059 Paint Blvd, Shippenville, PA 16254-Paint Booth Area-On or about January 30th, 2020 and times prior thereto: An employee wore a half mask air-purifying respirator (3M brand 5300 series respirator with organic vapor cartridges) during painting operations on truck and trailer parts. The employer did not establish portions of a respiratory protection program that covered medical evaluations and respirator maintenance and care. The Unique Fabrications, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(c)(2)(ii) which was contained in OSHA inspection number 1136188, citation number 1, item number 4b and was affirmed as a final order on May 16th, 2016 , with respect to a workplace located at 19059 Paint Blvd, Shippenville, PA 16254 .
