29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a jobsite located at 4610 Northeast 85th Terrace, Lot 134, Kansas City, Missouri 64119, an employee was exposed to a serious fall hazard. An employee was installing guttering without adequate protection from falling by the use of  guardrail systems, safety net systems, or personal fall arrest systems. The employee was exposed to a fall distance of 19 feet 6 inches to the concrete below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a jobsite located at 4610 Northeast 85th Terrace, Lot 134, Kansas City, Missouri 64119, an employee was exposed to a serious fall hazard. An employee was accessing a residential roof with a Werner portable ladder. The portable ladder rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. The fall hazard distance was measured at 10 feet 9 inches to the concrete below.
