29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about January 24, 2020 a suspended metal junction box used as a power source for the various equipment (e.g.: saws, drills, etc.) in the warehouse area of the facility was not installed according to listing and labeling directions. The box (which contained knock-outs) was not mounted to a fixed surface.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about January 24, 2020, adequate strain relief was not provided for the flexible cord of a suspended junction box. The fitting and/or connector that was used to attach the device to the flexible cord was not properly connected, and thus had the potential of exposing employees to an electrical hazard should the wiring get damaged and/or be inadvertently pulled out of the device.
